Key Responsibilities\n

• Conduct thorough psychosocial assessments through structured interviews and observations to evaluate the emotional, social, and environmental factors that affect patient recovery.

• Deliver compassionate counseling and emotional support to patients and their families, guiding them through adjustment issues and promoting mental well-being.

• Collaborate effectively with our interdisciplinary rehabilitation team to develop comprehensive, individualized treatment plans that address both psychological and social needs.

• Help patients access essential community resources, including but not limited to support groups, vocational rehabilitation services, and housing assistance programs.

• Educate patients and their families about recovery resources, planning for post-rehabilitation services to ensure a smooth transition to independent living.

• Participate actively in discharge planning, facilitating patients’ transitions back to home or other necessary care settings, coordinating with external agencies as needed.

Qualifications\n

Compensation and Benefits\n

We offer a competitive annual salary ranging from USD 75,000 to 100,000, commensurate with experience and qualifications.
