RN - Case Manager

Job OpportunityThis position is available at an acute care facility located in Laguna. The role requires a candidate to float among departments and work within a team-oriented environment dedicated to providing high-quality patient care.ScheduleShift: Day, 5 days per week (08:00 AM - 04:30 PM)Weekend Requirement: Variable, with operations occurring 7 days per weekDays Off: Vary; consecutive days off will be arranged whenever possibleJob ResponsibilitiesManage a caseload of 20-23 patients, providing effective case management and discharge planning.Conduct thorough assessments within 24 hours of patient admission.Coordinate post-acute care needs and treatment plans in collaboration with a multidisciplinary medical team.Document all care delivery and patient responses accurately according to organizational standards.Educate patients and caregivers to facilitate smooth discharge processes.Respond to various patient needs across all age groups and diagnoses, with an emphasis on adult and geriatric care.QualificationsExperience: Minimum of 1 year of acute care experience required.Licensure: Valid California state license required.Certifications: BLS, MSW or LCSW required.Travel Experience: Prior travel experience is necessary.Skills: Proficient in care coordination, communication with interdisciplinary teams, and familiarity with the Epic charting system.Additional InformationThis position is not suitable for candidates specializing solely in behavioral health or emergency department settings.
