Responsibilities

• Provide direct and indirect skilled nursing care to residents under the supervision of a Registered Nurse.
• Collect data on residents’ physical and psychosocial status and communicate findings to the RN and Interdisciplinary Team.
• Contribute to development of an age-appropriate, culturally relevant plan of care, involving residents/families and the healthcare team to achieve outcomes.
• Monitor and revise plans of care as needed based on resident outcomes; coordinate referrals to ensure comprehensive care.
• Maintain current nursing knowledge and participate in education/in-service activities; mentor and orient other staff as needed.
• Demonstrate safe clinical practice for adult and geriatric residents and adhere to safety, infection control, and facility policies.

Requirements

• Active New York Licensed Practical Nurse licensure
• BLS certification (American Heart Association or RQI)
• Minimum 1 year of LPN experience in a long-term care facility
• Ability to work a rotating 5x8 schedule (days and nights)
• Capability to provide care to adult and geriatric residents
• Holiday coverage required
• Must be able to participate in the orientation schedule (Tuesday/Thursday for full-shift orientation)
