RN Case Manager: Discharge & Care Coordination

Hillsboro Medical Center seeks an experienced RN case manager to analyze patient records for quality assessment and discharge planning. You’ll coordinate discharge plans, collaborate with hospitalists and care teams, and prepare documentation for insurers to authorize post-discharge services. Required is an RN license with an associate degree; a BSN/MSN and post-discharge planning experience are preferred. Strong communication, organization, and problem-solving skills are essential in a #J-18808-Ljbffr Tuality Healthcare
