Nurse Care Coordinator

Job Summary and Responsibilities As our Care Coordination Nurse at Yavapai Regional Medical Center, you will lead comprehensive care progression and discharge planning for our diverse inpatient population (birth to adult). Leveraging your nursing expertise, you'll orchestrate the delivery of healthcare and social services, balancing patient/family needs with cost‑effective resources. This role is instrumental in advocating for patients, addressing complex factors such as spiritual needs and socioeconomic barriers, and ensuring seamless, equitable transitions of care. Every day you will conduct and update discharge planning assessments, facilitate multidisciplinary collaboration for timely transitions to post‑acute care settings, oversee implementation of discharge plans, optimize inpatient resource utilization, identify and mitigate readmission risks, and provide patient education and advocacy, while adhering to hospital policies and promoting cost‑effective, quality outcomes. To be successful, demonstrate critical thinking, problem‑solving, professional communication skills, thrive in a fast‑paced, self‑directed environment, prioritize effectively, and collaborate seamlessly within the Care Coordination Team. Complete and document a discharge planning assessment on identified patients or upon request; reassess and update plan as appropriate. Facilitate multidisciplinary discharge planning with relevant health team members, patients, and post‑acute providers. Oversee and evaluate implementation of the discharge plan. Collaborate with the multidisciplinary team to ensure progression of care and appropriate use of inpatient resources. Ensure timely transitions to appropriate post‑acute services based on clinical needs. Schedule The typical hours are Monday through Friday, 8:00 AM to 4:30 PM with rotating weekends including shift differential. Average one weekend per month, variable by need. Required Two years of acute hospital clinical experience or a Master’s degree in Case Management or Nursing in lieu of one year experience. Graduate of an accredited nursing school. Registered Nurse: AZ or Compact license. American Heart Association BLS within 90 days. Knowledge of managed care and payer environment. Critical thinking and problem‑solving skills. Effective collaboration with multiple stakeholders. Understanding of utilization management and case management program integration. Team player; assist other members when needed. Knowledge of CMS standards and requirements. Preferred Bachelor’s Degree in Nursing (BSN) or related field. Five years of nursing experience. Certified Case Manager (CCM) or Accredited Case Manager (ACM‑RN). Pay Range $39.18 - $58.28 per hour. #J-18808-Ljbffr
