Nationwide, Aramark SeniorLIFE Dietitians ensure that the nutritional needs of the residents are met, and that all documentation is complete and accurate. Plans and administers activities related to resident nutrition care, complying with the standards established by Aramark SeniorLIFE, regulatory agencies, and clients. Establishes and maintains effective multidisciplinary relationships to provide a unified approach to resident care. \n Conduct comprehensive nutrition assessments, reassessments, and follow-up evaluations using the Nutrition Care Process (NCP) and evidence-based practice guidelines. \n Develop, implement, monitor, and modify individualized nutrition care plans based on each patient's medical condition, nutritional status, laboratory values, medications, and clinical course. \n Provide medical nutrition therapy for a diverse inpatient population, including medical, surgical, critical care, cardiac, oncology, neurology, orthopedic, renal, gastrointestinal, and other specialty patient populations. \n Assess, recommend, and monitor enteral and parenteral nutrition support, including evaluation of nutrient requirements, formula selection, tolerance, and ongoing monitoring. \n Identify patients at risk for malnutrition and perform comprehensive malnutrition assessments in accordance with current clinical practice guidelines. \n Provide individualized nutrition education and counseling to patients, family members, and caregivers using techniques appropriate to health literacy and readiness to learn. \n Collaborate closely with physicians, advanced practice providers, nurses, pharmacists, speech-language pathologists, case management, and other members of the interdisciplinary healthcare team to optimize patient outcomes. \n Participate in interdisciplinary rounds, discharge planning, and patient care conferences as appropri \n Navitas Healthcare Job ID 26-10619.
