Learn more at: uncfamilymedicine.org Summary The Licensed Practical Nurse (LPN) will primarily support the UNC Family Medicine Center in Chapel Hill and is not expected to routinely float to other clinic locations. Working alongside physicians, advanced practice providers, nurses, residents, and other members of the care team, the LPN plays an integral role in delivering high-quality, patient-centered ambulatory care in a collaborative academic practice. The role includes a combination of direct and indirect patient care, with approximately 50% of responsibilities focused on indirect patient support, including care coordination, telephone triage (within scope), patient education, follow‑up communication, documentation, and assisting with care plans. The remaining responsibilities include providing direct clinical care while meeting the physical and psychological needs of patients in accordance with the plan of care, physician orders, and nursing and department policies and procedures. Schedule: Monday-Friday, 8:00 a.m. - 5:00 p.m. Qualifications Qualified candidates must be licensed as a Practical Nurse (LPN) in North Carolina (or eligible for licensure) with a current BLS certification and a minimum of two years of LPN experience. While previous ambulatory or family medicine experience is preferred, we also welcome candidates with inpatient, long-term care, or other outpatient nursing backgrounds who are interested in transitioning into family medicine. Success in this role requires strong communication skills, sound clinical judgment, and the ability to collaborate effectively with patients, families, providers, and interdisciplinary care teams. Responsibilities Contributes to the development and maintenance of a current, individualized, goal-oriented plan of care and consults with other health professionals to assure that the assessed needs of patient/caregiver are met. Observes, assesses, documents, and reports patient response to treatments/therapies and teaching, and adjusts priorities of care according to patient/caregiver needs. Collects, documents, and reports (to responsible MD or RN) data regarding patient's biophysical, psychosocial, environmental, self-care, educational, and discharge planning needs. Participates in unit, service, and/or department enrichment activities which contribute to positive outcomes such as assigned committees, CQI activities/groups, and the orientation and education of other personnel. Provides a broad range of treatments/therapies to patients including, but not limited to, respiratory treatments, Oral/NG/NT suctioning, and tracheostomy care, administration of medications, observation of cardiac monitors, respirators, etc. and reporting of any changes, assistance to physician with special procedures, monitoring of intravenous infusions, performs point of care testing, performs routine laboratory work, changing/irrigating catheters, specialized sterile dressings, and patient/caregiver teaching based upon the established teaching plan. Provides assistance to patients and families during admission to the clinical area, orientation to the hospital/clinic experience, and discharge. Provides support to patients and families. Verbally de‑escalates frightened or potentially aggressive patients. Facilitates age appropriate coping strategies for patients. Education Requirements Requires graduation from a state accredited program of practical nurse education. Licensure and Certification Requirements Licensed to practice as a Practical Nurse (LPN) in the state of North Carolina. Requires BLS. Professional Experience Requirements Requires two (2) years of previous experience as an LPN.
