NURSE PRACTITIONER

Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations in patient homes. You will conduct detailed histories and physicals, reconcile medications, identify care gaps, and develop evidence-based care plans that support value-based care. Using Signify’s technology platform, you’ll document visits, share insights with primary care teams, and help coordinate services for vulnerable populations.
