Occupational Therapist OT Home Health

Empath Home Health , a member of Empath Health is seeking a compassionate Home Health OT (Occupational Therapist) to work with our Home Health team to deliver individualized care to patients in their homes in Sarasota county.

Locations: Home health visits in Sarasota county (including North Sarasota, Longboat Key, South Sarasota, Siesta Key, Laurel, Osprey, Nokomis , and surrounding areas).

Schedule : Full-time; M-F (40 hours per week)

What You’ll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled occupational therapy services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You’ll Need

• Active Florida OT license
• Minimum of 1 year experience as a licensed Occupational Therapist (OT).
• Home health care experience strongly preferred.
• Prefers experience using Homecare Homebase (HCHB) EMR
• Strong assessment, critical thinking, and patient education skills
