Home Health Speech Language Pathologist-PRN

Overview As a Speech Language Pathologist with CenterWell Home Health, you will provide personalized speech and language therapy to patients in their homes, helping them regain strength, mobility, and independence. Responsibilities Evaluate, direct, and provide speech/language pathology services to patients in the home or facility. Participate in the development and periodic review of the Plan of Treatment and Plan of Care. Use professional skills and judgment to assess and treat disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize the effects of such disorders. Administer and interpret diagnostic tests and therapeutic treatments, including audio‑logic screening. Observe, record and report changes in the patient's condition and response to treatment to supervisor and/or the physician. Provide instruction and training to patients in the use of alternative communication systems when appropriate. Provide counsel and instruction to patients, families and healthcare staff. Maintain and submit required documentation and written reports of evaluations, visits, summaries, care plans, care coordination activities and progress reports. Participate in care coordination activities and discharge planning. Maintain the highest standards of professional conduct concerning confidential information. Required Qualifications Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA. Minimum six months of experience as a speech therapist / speech language pathologist. Current and unrestricted license. Current CPR certification. Valid driver’s license, auto insurance, and reliable transportation. Preferred Qualifications Home Health experience. Compensation & Schedule Pay Per Visit/Unit Rate: $49.29 – $67.82. Scheduled Weekly Hours: 1 hour. Pay Range for full‑time (40 hours per week) employment: $76,900 – $105,800 per year, varying by location and individual qualifications. TB Statement: This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB. Driving Statement: This role is part of Humana's driver safety program and requires a valid state driver’s license with personal vehicle liability insurance.
