Job Opportunity

Position Overview

We are seeking a skilled Healthcare Professional to provide in-home patient care. The ideal candidate will have experience in infusion therapy, a background in home health care, and possess essential nursing skills necessary for a variety of medical conditions.

Required Skills and Experience

• Infusion therapy experience is a must.
• Minimum of one year of skilled home care experience (private duty nursing experience alone is not acceptable).
• OASIS training and experience is required.
• Proficient in general medical diagnoses and high-risk diagnoses, including:
  
  
  • Heart Failure (HF)
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Myocardial Infarction (MI)
  • Pneumonia
  • Stroke
  • Post-operative care
• IV skills and wound care expertise.

Job Responsibilities

• Perform cardiopulmonary assessments and basic wound care.
• Administer ABI compression dressing and wound vac dressing.
• Provide care for various medical devices, including PleurX and Aseptd drains.
• Change and manage Foley catheters, perform staple and suture removal.
• Utilize glucometers and pro-time machines for patient assessments.
• Carry out Subcutaneous (SQ) and Intramuscular (IM) injections.
• Conduct lab draws via venipuncture and manage IVs, including PICC line or Midline care.
• Access and deaccess SQ ports as needed.

Work Schedule

The position requires flexibility in scheduling, with varying day and shift hours based on the needs of the Home Care office. Potential schedules include:

• Monday - Friday, 8 AM - 5 PM
• Monday - Friday, 9 AM - 6 PM
• Thursday to Monday or Friday to Tuesday shifts
• Alternating weekend shifts

Qualifications

• Valid driver's license and access to a vehicle for home visits.
• COVID-19 vaccinated or with an exemption on file.
• Flu shot or declination required, along with mandatory mask-wearing while at work.
• Must have current patient care experience within the last three months.

Technology and Documentation

Candidates will be provided with a laptop, MiFi for internet access, and portable printers. Documentations will be managed using BayCare Forms, BayCare Scheduling, and Ace Timesheet systems.

Additional Information

This role includes skilled home visits, averaging 6-8 visits per day, serving a patient demographic that includes adults and geriatrics. The daily patient census is expected to range between 400-1000 across locations.
