Inpatient Care Manager - MSW

Role: Inpatient Care Manager - MSW Responsibilities Assess, plan, implement, and evaluate individualized patient care and discharge plans across the care continuum. Coordinate safe and timely discharges, including securing authorizations, arranging transportation, and facilitating transfers. Monitor patient progress, participate in multidisciplinary rounds, and address barriers to discharge. Advocate for patients and families, addressing clinical, educational, and psychosocial needs. Collaborate with healthcare teams to develop and execute care strategies, ensuring patient preferences are incorporated. Manage resources to minimize unnecessary utilization and support performance improvement initiatives. Educate and assist patients and families in self‐management and adherence to care plans. Maintain knowledge of payer benefits, reimbursement, and coordinate communication with providers and families. Qualifications & Skills Master’s Degree in Social Work. Less than 1 year of experience in an MSW internship program. Strong ability to interact with diverse populations and prioritize care planning. Proficiency in computer applications, analytical tools, and EHR systems like EPIC (preferred). Proven leadership skills. Knowledge of Medicare, Medicaid, and payer policies (preferred).
