Responsibilities Provide direct patient care in the home setting, including assessments, treatment, and care planning. Administer medications and treatments as prescribed by physicians. Monitor patient progress and report any changes in condition to the healthcare team. Educate patients and families on health management and preventive care. Maintain accurate and timely documentation in accordance with company policies and regulatory requirements. Collaborate with other healthcare professionals to coordinate patient care and services. Adhere to infection control and safety standards to protect patients and staff. Requirements Valid Registered Nurse (RN) license in the state of Michigan. Minimum of 1 year experience in nursing, preferably in home health, hospice, or related fields. Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills. Ability to work independently and manage time effectively. Reliable transportation and willingness to travel within Wayne County, MI.
