Location: Southern Nevada State Veterans Home (Boulder City, NV) Pay Rate: $29.44 - $34.45, depending on experience Shift: 6:00am - 2:30pm- Days of the week are TBD

Position Overview

Reliable Health Care Services is seeking a dedicated and experienced Licensed Practical Nurse (LPN) to serve as a Medication Nurse at the Southern Nevada State Veterans Home in Boulder City, Nevada. This position provides direct, hands-on nursing care to residents within a 24-hour skilled nursing and rehabilitation environment operated by the Nevada Department of Veterans Services. Under the direction of a Registered Nurse, the LPN administers medications, performs therapeutic treatments, provides specialized wound care, and actively participates in care planning and resident assessment.

Responsibilities

  • Provide direct nursing care to residents
  • Administer prescribed medications safely and accurately; observe the five rights of medication administration and document promptly in the MAR/EHR
  • Perform therapeutic nursing procedures and treatments such as wound care, dressing changes, catheter care, and other physician-ordered interventions
  • Monitor, assess, and report resident responses to medications and treatments; identify and report changes in condition to the supervising RN or physician
  • Participate in the MDS (Minimum Data Set) and individualized care planning process to ensure resident needs are accurately assessed and addressed
  • Communicate effectively with residents, families, visitors, and interdisciplinary team members to promote a supportive and resident-centered care environment
  • Ensure security, accountability, and accurate documentation of controlled substances; promptly report discrepancies or variances per policy
  • Support infection prevention and control practices during medication preparation, administration, and wound care activities
  • Provide education to residents regarding medication regimens, treatment plans, and wellness strategies
  • Collaborate with the consulting pharmacist and clinical team to ensure timely medication availability and compliance with psychotropic and PRN medication regulations
  • Assist with shift handoff communication and interdisciplinary rounds; maintain continuity of care across shifts and disciplines
  • Supervise and support CNAs as delegated, reinforcing professionalism, resident rights, and quality of care standards
  • Respond appropriately to emergency situations, initiate life-saving measures, and notify emergency services as required
  • Maintain medication and treatment rooms/carts in accordance with infection control and safety standards; monitor medication storage temperatures and expiration dates
  • Participate in facility-wide quality assurance initiatives, medication audits, and training programs to promote ongoing professional development

Qualifications

  • Current Nevada LPN license in good standing
  • Minimum 1 year of recent nursing experience in a long-term care, skilled nursing, or geriatric setting
  • Current BLS certification (American Heart Association required)
  • Thorough knowledge of medication administration principles, documentation standards, and infection control practices in long-term care
  • Familiarity with CMS care standards; participation in the MDS and care plan process preferred
  • Ability to work a variety of shifts, including evenings, nights, weekends, and holidays, in a 24-hour facility
  • Strong clinical judgment, teamwork, and communication skills with the ability to maintain professionalism under pressure
  • Must meet background, health screening, and immunization requirements in accordance with Reliable and facility policy
