Key Responsibilities\n- \n
- Deliver individualized psychosocial support through counseling, education, and advocacy, tailored to the unique needs of each patient and their families. \n
- Document and monitor patient progress through electronic medical records, ensuring accurate communication of care strategies and outcomes. \n
- Engage with interdisciplinary teams to proactively address patient needs, preemptively identifying challenges that may impact the effectiveness of care plans. \n
- Facilitate smooth transitions of care by coordinating with various service providers, ensuring patients are fully informed and supported throughout their journey. \n
- Stay informed about changes in governmental and private payer networks, employing this knowledge to assist patients in navigating their options for care. \n
- Advocate for patient rights by guiding families through the healthcare system and ensuring access to necessary protective services when vulnerable situations arise. \n
- Assess and report any suspected abuse or vulnerabilities in accordance with legal and ethical guidelines. \n
- Participate in proactive strategies aimed at reducing hospital readmissions through careful patient management and follow-up. \n
- Organize and lead transition conferences to assist patients and families in understanding the next phases of care. \n
- Utilize analytical tools and risk assessment methods to identify patients at risk for readmission, ensuring timely and appropriate interventions. \n
- Fulfill additional responsibilities as assigned to support the overall mission of the organization. \n
- Provide ongoing support and counseling, remaining a consistent source of assistance for patients and their families. \n
Our focus is on serving a diverse range of patients, including those with acute mental health crises, chronic conditions requiring long-term management, and those facing terminal illnesses. The ideal candidate will be adept at interacting with patients from varied backgrounds, necessitating a keen awareness of cultural sensitivities as well as the ability to tailor services to meet individual needs.
Qualifications\n- \n
- Master's or Doctorate degree in Clinical Psychology, Counseling, or Clinical Social Work from an accredited program. \n
- Possession of a relevant independent license (LCSW, LMFT, LPC) in the state of Oregon or whichever state you reside in. \n
- Proven ability to efficiently utilize electronic medical record systems and maintain comprehensive documentation. \n
- Excellent interpersonal and electronic communication skills, particularly in collaborative settings. \n

- Experience managing a high caseload and interfacing effectively with large healthcare providers and associated support staff. \n
- A demonstrated passion for mental health and the ability to create a positive impact in patients’ lives within the spectrum of behavioral health care. \n
