Registered Nurse (RN) – Home HealthFreedom at Home is a Veteran-Owned Home Health Agency and we're looking for compassionate, motivated Registered Nurses (RNs) to join our growing team.If you're looking for a company that values its clinicians, promotes work-life balance, and provides the support you need to deliver exceptional patient care—we'd love to meet you.What We OfferCompetitive payFull-Time, Part-Time, and PRN opportunitiesPaid mileageFlexible schedulingSupportive leadership teamPositive, clinician-focused cultureOpportunities for growth as we expandThe chance to make a meaningful difference in patients' lives every dayResponsibilitiesPerform skilled nursing visits, assessments, admissions, recertifications, resumptions of care, and discharges in patients' homes.Develop, implement, and update individualized Plans of Care.Complete OASIS assessments and clinical documentation accurately and on time.Monitor patient progress and communicate changes with physicians and the interdisciplinary team.Educate patients and caregivers on disease management, medications, treatments, and safety.Coordinate care with physicians, therapists, home health aides, and other healthcare professionals.Supervise Home Health Aides in accordance with state and federal regulations.Promote safe, compassionate, high-quality patient care while helping patients remain safely at home.QualificationsGraduate of an accredited Registered Nursing (RN) program.Current, unrestricted Ohio Registered Nurse license.Current CPR certification.Minimum of one year of nursing experience required.Home Health experience is preferred but not required.OASIS experience is a plus.Valid driver's license, reliable transportation, and auto insurance.Why Freedom at Home?At Freedom at Home, our clinicians are our greatest asset.
