Schedule

8am to 4pm, Monday - Friday with some on call and weekends required.

Job Description

The Registered Nurse will provide high-quality patient care by assessing patient and caregiver needs and developing care plans in accordance with state and federal regulations. The RN will actively manage caseloads, complete OASIS documentation, and implement medical plans of care to ensure safe, effective quality care.

Responsibilities

• Assess patient and caregiver needs using the nursing process to provide quality care.
• Develop realistic care plans in collaboration with patients and caregivers.
• Serve as Case Manager, actively managing patient cases and visit schedules.
• Complete OASIS documentation accurately and timely.
• Implement medical plans of care and nursing interventions as per best practices.
• Maintain up-to-date knowledge of high-tech nursing skills and infection control standards.
• Collaborate with LPNs to ensure adherence to care plans and quality patient care.
• Evaluate the effectiveness of care plans and make necessary adjustments.
• Document nursing care accurately to facilitate quality healthcare team integration.
• Educate patients and caregivers on medication side effects, treatment options, and discharge instructions.
• Coordinate patient care with interdisciplinary teams to support effective treatment.
• Participate in monthly peer review audits and case conferences.
• Advocate for patient care issues to ensure quality care tailored to patient needs.

Required Experience / Certifications / Licensure

• OASIS experience is required.
• Current RN license in the state of practice.
• Experience in home health care preferred.
