Certifications:

• Active Ohio Pharmacist license

Mandatory Experience:

• Pharmacist 
• 1+ year of inpatient hospital pharmacy experience.
• Experience with Epic Willow Inpatient is preferred
• Must have relevant experience with pharmacokinetic dosing (specifically vancomycin) and warfarin management.
