Registered Nurse, Per Diem

We deliver individualized, family-centered support—partnering with families and caregivers and applying an interdisciplinary approach so every patient can reach their fullest potential. The Role We're looking for a registered nurse to support adults in their homes and communities. This is a hybrid, per diem position: most of the work, documentation, coordination, and care planning is done remotely. In-person time is reserved for what truly requires it: patient evaluations, assessments, and CHHA determinations. All clinicians are expected to participate in the firm's accreditation activities and surveys to maintain our quality standards. What You'll Do Conduct comprehensive health and psychosocial evaluations, and monitor each patient's condition. Coordinate total patient care—health promotion, prevention, service coordination, and teaching. Determine the need for and frequency of CHHA services based on patient and home assessments. Develop and implement individualized nursing care plans; evaluate their effectiveness on an ongoing basis. Complete admission, transfer, re-certification, resumption-of-care, and discharge documentation. Submit clinical and progress notes weekly, plus a daily tally of patient visits. Consult with attending physicians on care plan changes; loop in your supervisor as needed. Present records to the Clinical Record Review Committee and participate in case conferences. Explain nursing and firm services to patients and families as part of care planning. Participate in firm accreditation activities and surveys. What You Bring Active NJ Registered Nurse license in good standing. Mid-level clinical experience; home care or I/DD experience preferred. Comfort working independently in a remote-first workflow, with reliable transportation for in-person visits. Strong documentation habits and clear communication with patients, families, and physicians. #J-18808-Ljbffr
