RN UNIT MANAGER

Job Summary: Polaris Healthcare and Rehabilitation Center, a 100-bed facility located at the Milford Wellness Village, seeks a compassionate and experienced RN Unit Manager to manage the unit, utilize the nursing process to assess, plan, implement, evaluate resident care, and ensure high quality, compassionate care. Responsibilities: Utilize the nursing process to assess, plan, implement, and evaluate resident care. Assess signs and symptoms indicating changes in the patient's condition. Document the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions. Perform interventions according to identified priorities, plan of care, hospital policies, and patient care outcome standards. Revise the plan of care according to evaluation, changes in the medical plan of care, and effective/ineffective nursing interventions. Use clinical judgment in evaluation activities to meet patient care needs of an assigned unit/floor, including establishing priorities. Qualifications: Must be a licensed RN with a valid license. Familiarity with Federal, State, and local long‑term care requirements. Two years of professional nursing experience. Additional Information: Full‑time position. Monday‑Friday schedule, days only.
