About the Position

• Conduct initial and ongoing assessments, monitor symptoms, and adjust the plan of care as needed .
• Case manage a dedicated caseload, acting as primary RN and main point of contact for patients and families.
• Coordinate with providers, social work, chaplains, and other team members to ensure holistic hospice care.
• Document all visits, interventions, and changes in condition using HCHB (Homecare Homebase) EMR​.
• Educate patients and caregivers on medications, symptom management, and end-of-life care expectations

Requirements

• Minimum 1 year of recent  case management experience in home health or hospice (must be willing to take and manage a caseload).
• Reliable personal vehicle; comfortable traveling throughout the day to patient homes (this is  not inpatient).
• HCHB (Homecare Homebase) EMR experience is required​.
• Prior travel RN experience is required.
