Requirements: • Board Certifications Req’d: Family Medicine Will accept IM – if sees all ages• State Licenses: NH required• Certifications: BLS• DEA Req’d?: yes• Years of Experience Req’d: 1Credentialing Timeframe: 90 days—trying to push a faster start since there shouldn’t be any privileges with being OP clinic only Willing To License?: noName Clear: name and npi #Travel: mileage hotel rental car flight Shift Info: Day 5x8-Hour 08:00 - 17:00
