CERTIFIED HOME HEALTH AIDE-CHHA

Responsibilities: Receives and gives report on resident status at the beginning and end of each shift and as needed. Notifies the clinical team of any changes in resident physical and/or mental status. Provides personal care to residents per their individualized plan of care. Completes prevention and restorative care including, but not limited to, ambulation, transfers, bathing, grooming, and all other ADL care. Documents all pertinent information regarding care as assigned. Assists in turning, positioning, and repositioning of residents as outlined in the resident's service plan. Communicates effectively with the residents, families, visitors, members of the health care team, and other disciplines Requirements: NJ Certified Home Health Aide (CHHA) certification must be current and active Ability to work in a collaborative work environment while being accountable for individual goals Ability to respond to and work with a mature population, contributing to a warm, caring and supportive environment for the enjoyment of the residents/clients and to promote this philosophy to all team members Must be available to work weekends, as scheduled Benefits: 401(k) Dental insurance Flexible schedule Flexible spending account Health insurance Paid time off Referral program Vision insurance Work Location: In person EOE: FellowshipLIFE is an equal opportunity employer.
