Post-Acute Care Liaison We are a nonprofit skilled nursing facility in Brooklyn committed to delivering exceptional care to patients transitioning from hospitals, individuals with chronic illnesses, and older adults requiring rehabilitation. Our facility operates at the intersection of compassionate care and business excellence, providing top-tier clinical programs while ensuring sustainable growth. We are seeking a results-driven professional who thrives on generating revenue but also understands that every referral represents a patient and a family whose care is paramount. The Post-Acute Care Liaison will be responsible for driving admissions, primarily from Medicare-funded sources, by cultivating and expanding hospital referral relationships. This is a high-impact role for a professional who combines a business-minded approach with an unwavering commitment to quality care. Success requires deep knowledge of the skilled nursing landscape, proven sales or business development expertise, and the ability to translate market opportunities into measurable admissions and revenue growth. Key Responsibilities Develop, implement, and execute aggressive sales strategies targeting hospital discharge planners, case managers, and other referral sources. Identify and pursue high-value referral opportunities to maximize Medicare admissions and facility revenue. Maintain deep knowledge of skilled nursing services, rehabilitation programs, and patient care protocols to effectively position the facility in a competitive market. Build and sustain long-term relationships with key decision-makers in hospitals, healthcare networks, and community organizations. Collaborate closely with clinical leadership to ensure smooth admissions processes and superior patient experiences. Track and report referral trends, admissions data, and market performance to refine strategy and achieve revenue goals. Represent the facility at professional and community events to raise visibility and drive business development opportunities. Actively contribute to the facility's culture of care, demonstrating accountability, professionalism, and empathy in all interactions. Required Qualifications Minimum of 35 years of experience in sales, business development, or marketing specifically within skilled nursing or post-acute care. Demonstrated track record of achieving and exceeding revenue or admissions targets in a competitive environment. Deep understanding of Medicare, hospital discharge processes, and referral networks. Proven ability to build and sustain relationships with healthcare executives, case managers, and hospital staff. Exceptional communication, negotiation, and presentation skills; able to influence decision-makers with professionalism and authority. Highly organized, self-motivated, and results-driven with a strong business acumen. Patient-focused mindset; able to balance revenue objectives with the mission of providing compassionate care. Compensation Base salary: $140,000$150,000, commensurate with experience. Performance-based bonuses tied directly to admissions and revenue growth. This role is ideal for candidates who are not only business-savvy but also genuinely invested in improving the patient experience every day of the year. Note: This position is for experienced skilled nursing professionals only. Candidates without a proven track record in skilled nursing admissions, post-acute care sales, or hospital referral development will not be considered.
