LPN - Home Health

Community Health LPN

This is a 13-week contract position.

8:00 AM - 4:30 PM, 8-hour days, 40 hours per week.

The Community Health LPN provides and documents nursing care for the sick and injured in home settings. The LPN works to promote, maintain, or restore health, prevent illness, and facilitate rehabilitation. In collaboration with healthcare providers, professionals, and community representatives, the LPN coordinates care and evaluates individual and family responses to interventions.

Responsibilities:

• Provide and document nursing care in home settings.
• Promote and restore health while preventing illness.
• Coordinate care with healthcare and community representatives.
• Evaluate responses to nursing interventions.
• Function independently under the guidance of the Director.
• Utilize time management, communication, planning, and problem-solving skills in care provision.
• Exercise independent judgment and critical thinking in emergency situations.
• Maintain emotional stability in various settings.
• Conform to patient rights and customer service policies.

Required Experience and Certifications:

• One year of acute nursing care experience required, with hospital-based experience preferred.
• Experience in home health is required.
• Certification in Home Health, including proficiency with OASIS, is highly preferred.
• Experience with infusion therapy is preferred.
• Current Massachusetts LPN license is required.
• Active American Heart Association BLS or American Red Cross Certification.
• Valid driver's license and insurance, with the ability to drive in inclement weather.
• History of dependability in meeting work schedule demands.
