Contract (13 or 26 weeks) | Full-Time | Days | Monday–Friday, 8:00 AM–5:00 PM | Onsite
Key Responsibilities
- Provide physical therapy services to patients within an assigned geographic territory covering Walla Walla County, the lower portion of Columbia County, and into Umatilla County (up to 60 miles)
- Collaborate with a manager and interdisciplinary team as part of a geographic-based care team
- Evaluate and treat patients in the home health setting, developing and implementing individualized care plans
- Document patient care and progress using the electronic medical records system ( HCHB experience preferred )
Required Qualifications
- Active Washington State Physical Therapist license required
- Oregon Physical Therapist license required (candidates may begin with WA licensure while actively obtaining OR licensure)
- Minimum of 12 months of Home Health experience within the past 3 years
- No on-call requirements
Preferred Experience / Skills
- Experience with HCHB (HomeCare HomeBase) EMR platform
- Familiarity working within interdisciplinary home health care teams
