With modern tools, robust development pathways, and a culture grounded in diversity, equity, inclusion, and belonging, we are redefining what it means to work in community health.About the RoleVanna is seeking Integrated Health Coaches with a deep passion for supporting individuals with behavioral health challenges as they build fulfilling, connected lives within their communities. Coaches are responsible for locating and engaging members, building trusting relationships, and providing exceptional coaching, advocacy, and support throughout their recovery journey.This partnership begins with understanding each member's personal goals and connecting them to the right resources to achieve those goals. Coaches play a critical role in both physical and behavioral health coaching, collaborating closely with clinicians and specialists to ensure members receive coordinated, high-quality care.We believe strongly in the power of human-to-human interaction. Coaches meet members where they are within their communities, at their homes, and through phone, text, and email. Your insight and lived experience will be deeply valued as we continue improving the member experience, informed by the voices of our population and the communities that support them.Key ResponsibilitiesConduct initial outreach to eligible members through phone, virtual, and in-person interactionsEngage and build trust: Locate and engage members in their communities, establishing strong, collaborative relationships with members and their care teamsAchieve enrollment goals: Meet daily enrollment and engagement targets to support program growth and member impactSupport recovery and self-determination: Partner with members to develop, track, and achieve personal recovery goalsProvide role modeling and encouragement: Serve as a consistent, positive presence throughout each member's recovery journeyFacilitate wellness and coping strategies: Encourage healthy behaviors and coping strategies that promote physical and behavioral healthDeliver physical health coaching: Provide physical health coaching in accordance with Vanna training and program guidelinesDrive continuous improvement: Share feedback with Vanna's technology and operations teams to improve workflows and the member experienceMaintain accurate and timely documentation while complying with HIPAA, health plan requirements, and Vanna Health policiesRequired QualificationsMinimum of 2 years of experience working directly with individuals living with serious mental illnessExperience working within NYC communities and boroughsExcellent verbal and written communication skillsStrong organizational and time management skills with the ability to work independently in a remote environmentComfortable using technology and learning new software quicklyWillingness to become proficient in Google Workspace (Gmail, Calendar, Docs), Zoom, Slack, and other required technology platformsHigh school diploma or equivalentDriver's license and willing to use personal vehicle (for Long Island only)Preferred QualificationsBilingual fluency in SpanishCertified Peer Specialist TrainingExperience supporting individuals with Substance Use DisordersPrevious success working remotely; strong adaptability, organization, and proactive communicationLogistics & RequirementsAbility to commute to Manhattan for day 1 of training (travel cost will be reimbursed in accordance with Vanna's travel policy)Dedicated home office with a private, HIPAA-compliant workspace and reliable high-speed internet for long-term remote workAbility to travel throughout Brooklyn using public transportation to meet with members in the communityAbility to spend a significant portion of the workday in the field meeting members, as neededBenefitsFull health, dental, and vision coverage401(k) retirement plan availableAs part of our hiring process, candidates being considered for employment will be required to complete a background check and a drug screening, in compliance with applicable laws.
