Position Title: Licensed Practical Nurse (LPN) PRNBeacon Health Management is hiring a Licensed Practical Nurse (LPN) to work at Hazlehurst Court Care and Rehab providing both short term rehabilitation and long-term care.Position Summary:In conjunction with the Unit Director, the Licensed Practical Nurse utilizes a general understanding of the principles of Nursing in the development of and implementation of individualized Nursing Care Plans to ensure that the needs of the customers are met. He/she assists in the orientation of and supervision of nursing personnel, attends to the daily operations of the unit on per shift, unit level, and assumes responsibilities of a leadership role as needed. Licensed Practical Nurse (LPN) RESPONSIBLITIES:Takes an active role in direct customer careAdministers medication and/or treatment and documentation of same to customersResponsible for clinical documentation related to customer activityAssists in developing and implementing individual nursing care plan for each customer. Participates in care plan meetingsCounts and records class drugs when beginning and ending shiftCommunicates pertinent data to Unit Manger/Supervisor and/or PhysicianReviews and updates communication board; as well as monitors and addresses alerts frequently throughout shiftProvides for emotional and physical comfort and safety of customersAssist Physician in rounds and examination of customers, as neededSupervises and coordinates activities of Certified Nursing personnel and other LPN's in providing direct customer care in adherence to State, Federal and Corporate guidelinesEnhances education by attending all mandated in-service programsPuts Customer Services First: Ensures that customers and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals' needs and rights  EDUCATIONAL/VOCATIONAL REQUIREMENTS:1. The Licensed Practical Nurse must be a graduate of an approved school for Practical or Vocational Nursing and must be currently licensed by the State Board of Nursing.2.
