Home Health Licensed Practical Nurse (LPN)

Location: Stuart, FL & Surrounding Areas
Job Type: Full-Time | Field-Based
Compensation: $45,000–$55,000 Annually

About the Opportunity

Our client is an established and growing home healthcare organization seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join their clinical team serving patients throughout Stuart, Florida, and the surrounding communities.

This is a full-time, field-based position providing skilled nursing care to patients in their homes. The ideal candidate is a licensed LPN who is comfortable working independently, managing patient visits, and collaborating with a multidisciplinary healthcare team to deliver high-quality care.

Candidates with previous experience in home health, post-acute care, skilled nursing, rehabilitation, or geriatric nursing are strongly preferred.

Key Responsibilities

• Provide skilled nursing care to patients in their homes under the supervision and direction of a Registered Nurse (RN) and in accordance with established plans of care.
• Conduct patient visits throughout Stuart and the surrounding service areas.
• Monitor and document patients' vital signs, symptoms, and overall health conditions.
• Administer medications and perform prescribed treatments within the LPN scope of practice.
• Assist with wound care, dressing changes, catheter care, and other skilled nursing procedures as appropriate.
• Monitor patients for changes in condition and promptly communicate concerns to the supervising RN or appropriate healthcare provider.
• Educate patients and their families on medication management, disease processes, safety precautions, and ongoing care needs.
• Maintain accurate, timely, and thorough clinical documentation in accordance with agency policies and regulatory requirements.
• Collaborate with RNs, therapists, physicians, and other healthcare professionals to ensure continuity of care.
• Support patients transitioning from hospitals, rehabilitation facilities, or skilled nursing facilities into the home setting.
• Follow all applicable patient safety, infection control, and home healthcare regulations.

Qualifications

• Active and unrestricted Licensed Practical Nurse (LPN) license in the State of Florida required.
• Previous nursing experience in home health, post-acute care, skilled nursing, rehabilitation, or geriatrics strongly preferred.
• Experience providing care to elderly patients and individuals managing chronic illnesses is highly desirable.
• Strong clinical assessment, patient monitoring, and nursing documentation skills.
• Familiarity with wound care, medication administration, and chronic disease management.
• Ability to work independently in a field-based environment while maintaining effective communication with the clinical team.
• Strong organizational and time management skills, with the ability to manage multiple patient visits throughout the day.
• Experience using electronic medical records (EMR) systems preferred.
• Excellent interpersonal skills and a compassionate, patient-centered approach to care.
• Valid driver's license, reliable transportation, and willingness to travel locally to patient homes.
• Ability to meet applicable home health regulatory and clinical requirements.
