Job Summary:
The organization is seeking a Physical Therapist with specialization in Home Health in Portland, Oregon. The position involves providing therapy services to adult and geriatric patients in a home health setting, primarily in West Portland and surrounding areas. Candidates are required to bring their license on the first day of work.

Job Details:

• Location: Portland, Oregon
• Schedule: Days, 8 hours per day, minimum of 40 hours per week
• Required Qualifications: Current Oregon license; one year of skilled home health experience; BLS certification; familiarity with EPIC Remote Client for Home Health and Oasis documentation; reliable transportation.
• Preferred Qualifications: Experience with Wound Care PT; billing home health; OASIS documentation.
• Additional Information: The position involves working with adult and geriatric patients with a variety of orthopedic, neurological, and chronic conditions, utilizing skills in evaluation, assessment, gait training, wound care, and the use of assistive devices. The provider will need to adhere to facility policies regarding attire, charting, and patient care protocols. A good understanding of healthcare documentation systems and the ability to coordinate patient care efficiently is essential.
