This is an exciting opportunity to work alongside dedicated clinicians to meet the needs of clients in the community we serve.Home Health Registered NurseFull-time, Monday-Friday 8a-430pHome Health Nurse (RN)Stella Maris Voted #1 "Best Homecare Services" for 2023 by Baltimore Style Magazine**This position requires daily travel in the Howard County, Maryland region**ResponsibilitiesUnder the direction of the clinical manager, the Home Health Registered Nurse performs primary nursing care functions in conformance with professional standards of home health nursing practice.• Providing individualized care to patients in their home under the plan of care approved by the primary physician. The home health nurse coordinates all services provided for each assigned patient and family.• The Home Health Registered Nurse provides leadership and advocacy in the coordination of patient care with multiple clinicians, modalities, and community resources. They manage clinical resources and transition planning for patients within the assigned caseload from pre-admission through post-discharge, actively working to provide excellent medical care.• Explains Home Care and Hospice Home Care services to prospective patients/families accurately, thoroughly, and in an understanding manner. Explains insurance benefits and coverage of services, patient responsibilities and options for alternative means of payment• Develops the initial plan of care based on input from patient, family and physician. Requests appropriate disciplines for care• Reports admissions to Patient Care Coordinator within 24 hours and updates Hospice Team at weekly team meetings.• Documents accurately all admissions, follow-up visits and phone calls. Reviews chart for completeness and accuracy when case is closed.Requirements• RN licensure in the State of Maryland required• (2) years experience in home health required• OASIS Documentation experience required• EPIC experience preferred• Valid driver's license and reliable transportation• Commitment to Stella Maris philosophy of care, values, mission statement and organizational cultureMin USD $38.05/Hr.Max USD $57.05/Hr.Compensation decisions are made in alignment with internal equity, role scope, and market data to ensure fair and consistent pay practices. Most new hires are typically placed within the lower to mid-portion of the range; offers above that level are reserved for candidates with advanced or highly specialized qualifications.
