Job Description

• Conduct thorough evaluations of patients in either inpatient or outpatient environments and craft tailored treatment plans
• Deliver physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Work closely with nurses, physicians, case managers, and a diverse range of rehab professionals to elevate patient outcomes
• Engage and educate patients and families on treatment objectives, discharge planning, and at-home exercises
• Maintain comprehensive documentation of evaluations, treatments, and progress that aligns with facility and regulatory standards
• Engage in interdisciplinary care team rounds and conferences to ensure seamless care

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

• Must hold a state license as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory

Skills

• Prior experience is strongly preferred
