Director of Social Services – LMSW or BSW
Tallgrass Healthcare Campus is a full-service elder care community located in Junction City. The Social Services Director plans, organizes, and implements social service programs to meet the emotional, social, and psychological needs of residents in a 100-bed Skilled Nursing Facility within a CCRC setting. This role ensures compliance with federal, state, and local regulations while promoting resident rights and quality of life.
Job Responsibilities
· Plans, develops, organizes, implements and evaluates the social services programs.
· Contacts resident and establishes a positive relationship with residents. Maintains individual contact with each resident to aid, assist and/or counsel in regard to specific psychosocial needs.
· Completes and updates social histories for facility residents.
· Participates in discharge planning, development and implementation of social care plans.
· Involves the resident/family in planning social service programs when possible.
· Refers resident/family to consultants, therapists, social service agencies and ensures other resources are available in conjunction with appropriate staff and approval of the supervisor.
· Interacts with APS and other agencies on behalf of residents.
· Assists in coordinating admissions/transfers/discharges with other departments.
· Identifies the medically related social services and emotional needs of residents.
· Provides social services to residents requiring such services or makes referrals to Social Agencies.
· Conducts an initial social service assessment of all admissions, completes the social service section of the MDS, addresses social service needs on the plan of care, and maintains current and up-to-date social service progress notes on all residents.
· Processes all Medicare denial letters.
· Engages in various social service functions for independent and assisted living residents such as insurance questions, coordinating support groups, and serving as a liaison between independent living, assisted living and the Health Center.
· Assist with independent living, assisted living, and Health Center special functions, as needed.
· Assist in staff training and education.
· Helps resident and family orient to facility.
· Oversees all admissions related paperwork with the resident and/or responsible party including Medicare Part-A coverage and non-coverage procedures plus all non-medical related paperwork in an admissions file for each resident.
· Informs Medical Records of on-going daily census activity such as admissions, discharges, deaths, room transfers, changes in payment methods, and temporary and permanent apartment status.
· Maintains complete confidentiality regarding admissions information and resident records.
· Directs referrals from hospital social workers, physicians, or family members to marketing and admissions.
Job Qualifications
· Previous job experience preferred but not required
· Must be a Licensed Social Worker within the State of Kansas.
· Must have excellent time management skills and be able to work within deadlines
· Must be able to maintain a courteous, friendly, and helpful attitude with residents, visitors, and co-workers at all times
· Must be able to regularly lift and/or move up to 50lbs
