About the Role:
As a Care Coordinator, youll support individuals with complex needs by assessing, planning, and coordinating servicesprimarily for those with opioid use disorder. Youll connect clients to resources, monitor progress, and advocate for their continued care and recovery.
What Youll Do:- Conduct comprehensive client assessments
- Develop and manage individualized service plans
- Coordinate services and community resources
- Monitor client progress through regular contact
- Maintain accurate documentation and records
Who We Are Looking For:
- Bachelors degree in healthcare, psychology, social work, or related field
- 1+ year of care management experience (SMI, SUD, or similar populations)
- Valid Florida drivers license with clean record
