Oak River Rehab, a skilled nursing facility in Anderson, California, is seeking a dedicated and experienced MDS Coordinator to join our clinical leadership team.
Position: MDS Coordinator Location: Anderson, CA Pay: Starting at $45.00/hour and up, depending on experience
Requirements:
- Current LVN or RN license in good standing
- Experience with MDS assessments and care planning preferred
- Knowledge of Medicare, Medicaid, and reimbursement regulations
- Strong organizational and communication skills
- Ability to work collaboratively with interdisciplinary teams
Responsibilities:
- Coordinate and complete MDS assessments accurately and timely
- Ensure compliance with federal and state regulations
- Develop and update resident care plans
- Collaborate with nursing, therapy, dietary, and social services departments
- Monitor documentation and reimbursement processes
