Overview
Licensed Practical Nurse (LPN) – Long-Term Care travel assignment in South Burlington, Vermont. Start date: 07/28/2026. Duration: multi-week assignment. Weekly pay range: $1,902-$2,023 per week. Guaranteed hours: 40 per week. Shift: Evening and/or overnight. This is a travel - temporary assignment with Healthforce Healthcare LLC, working in a long-term care setting across eligible facilities.

Responsibilities
- Provide direct nursing care to residents under supervision, including medication administration and monitoring of vital signs
- Assess patient condition, identify changes in status, and communicate findings to the care team
- Perform routine nursing procedures, wound care, and hygiene assistance per care plans
- Document care activities accurately in patient records and ensure continuity of care
- Collaborate with licensed nurses, physicians, and interdisciplinary team to implement and update care plans
- Adhere to infection control practices, safety protocols, and patient privacy guidelines

Requirements
- Active LPN license valid in Vermont (or eligibility to practice in Vermont)
- Graduate of an accredited practical nursing program
- Experience or proven interest in long-term care or geriatrics
- Ability to work evening and/or overnight shifts
- Strong clinical skills, documentation accuracy, and ability to collaborate with a care team
- Eligible for travel assignment and able to commit to a multi-week shift schedule
