Job DescriptionJob DescriptionThe Part Time Private Duty RN will provide skilled nursing care in patients' homes, focusing on chronic illness management under the guidance of a physician and patient/caregiver input. This role involves comprehensive patient assessment, developing and implementing care plans, and maintaining accurate documentation using an Electronic Health Records (EHR) system. The nurse will collaborate with the home health team to ensure compassionate, culturally sensitive care that promotes patient comfort and safety. ResponsibilitiesConduct initial and ongoing patient assessments using OASIS data elementsDevelop, implement, and adjust individualized care plans with patient and physician inputDocument all observations and treatments accurately in compliance with agency policiesMaintain patient confidentiality in accordance with HIPAA regulationsProvide emotional and physical comfort considering patient rights and cultural backgroundCommunicate changes in patient condition and unusual reactions to physicians promptlyParticipate in team meetings, case conferences, and performance improvement activitiesAdhere to all agency policies and procedures at all times Required QualificationsMinimum 1 year of nursing experienceValid Nursing License in the state of Washington Preferred QualificationsStrong skills in patient assessmentKnowledge and compliance with HIPAA privacy regulationsProficiency in clinical documentation using EHR systemsExcellent communication and team collaboration abilitiesAdaptability to varied patient needs and environmentsEmpathy and commitment to patient-centered care_________________________________________________________________________________________________Ensuring your privacy is important to us at Haven Specialty Home Care.
