You will use assessment tools to manage the medical needs of a patient and identify other disciplines needed to maximize the patient’s outcome. You will be a patient advocate, and will be in communication with the primary Doctor, Clinical Team Manager, and assigned Registered Nurse, as the patient’s condition requires.
