In this role, you will assess, plan, coordinate, and manage care while helping members navigate resources, improve health outcomes, and overcome barriers to care.What You'll DoConduct comprehensive telephonic assessments and develop individualized care plans.Identify health risks, barriers, and opportunities to improve member wellness.Coordinate care, referrals, and transitions to appropriate clinical programs and resources.Apply clinical judgment to support members with complex and co-morbid conditions.Collaborate with multidisciplinary teams to achieve optimal member outcomes.Maintain compliance with regulatory requirements and company policies.ScheduleMonday-Friday schedules available between 7:00 AM - 6:00 PM CSTFlexible schedule selection within business hoursChoice of 30- or 60-minute unpaid lunchNo nights, weekends, or holidays currently required (subject to change based on business needs)Required QualificationsActive, unrestricted RN license in state of residenceWillingness to obtain a Compact RN license (company-paid)Minimum 5 years of clinical RN experienceAssociate Degree in Nursing (ADN)Ability to work within Monday-Friday business hoursPreferred Qualifications6+ months of Case Management or Utilization Management experienceCase Management Certification (CCM or equivalent)BSNIf you're a compassionate RN who enjoys helping members achieve better health outcomes while maintaining a strong work-life balance, we'd love to hear from you. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Our people fuel our future.
