Physical Therapist

The physical therapist is responsible for providing physical therapy services to clients of the agency. The primary function is the provision of direct patient care, including collaboration with other team members and participation in the development of the patient's comprehensive plan of care.

Rehabilitation equipment including but not limited to:

• splints
• orthoses
• prostheses
• wheelchairs
• ADL equipment
• Modalities
• Candidate required to be Home Health Certified

Requirement description:

• Client prefers 45 min or less commute from Tor Office (165 Tor Ct, Pittsfield, MA 01201)
• 2+ acute nursing care (hospital base) and Home Care experience [Required]
• Valid Driver License required for this need [Required]
• Must be able to work independently with minimal supervision/assistance
• Acute care experience [preferred]
• Certified home health experience including proficiency with OASIS required and NON-NEGOTIABLE [Required]
• Traveler perm address must be included in profile
• Traveler must have their own vehicle for this need
• Any/All RTO must be included in profile. ALL SCHEDULING REQUESTS MUST BE MADE AT THE TIME OF SUBMISSION. Facility will NOT approve scheduling requests at/after time of offer. Please obtain this information prior to submittal. Requests after offer may lead to rescinded offer
