Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings to create tailored treatment plans.
• Implement evidence-based physical therapy interventions that enhance mobility, strength, balance, and overall function.
• Engage in collaboration with a multidisciplinary team, including nurses, physicians, case managers, and fellow rehabilitation specialists to achieve optimal outcomes for patients.
• Empower patients and their families by educating them about therapeutic goals, safe discharge planning, and home exercise routines.
• Maintain meticulous documentation of assessments, treatments, and patient progress in line with facility protocols and regulatory requirements.
• Participate actively in interdisciplinary care team rounds and conferences to ensure a seamless continuum of care.

Education

• Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution.

Certifications

• Valid state licensure as a Physical Therapist .
• CPR certification is preferred.
• Any specialized certifications (neurological, geriatric) are advantageous but not mandatory.

Skills

• Previous experience is highly preferred.
