Salary: Starting at $56,000+ (based on experience)

Full-time, Salaried | Benefits after 90 days

Genesis Community Homes is looking for a dynamic, hands-on Care Coordinator to drive excellence in healthcare coordination for the individuals we serve, improve staff development, and program quality. What You’ll Own

Medical & Care Coordination

· Coordinate all medical appointments, records, and medication tracking for our individuals and ensure follow-through on physician recommendations

· Manage and maintain MARs, Lifetime Medical Reports, Current Medications lists and HRST updates within specified timeframes

· Attend ISP meetings with accurate and up to date information

· Serve as key liaison with providers, pharmacies, specialists and families

Training & Team Development

· Lead Medication Administration & CPR training programs

· Conduct audits, observations, and ensure regulatory compliance

· Partner with leadership to strengthen staff performance

· Design training that builds independence for the individuals we serve

Leadership & Operations

· Serve as a coach and mentor to Residential Supervisors and staff

· Drive compliance with PA ch.6400 & ODP regulations

· Lead program audits and improve systems

· Act as Certified Investigator / Incident Lead

· Help maintain safe, structured, therapeutic homes

What Makes You a Great Fit

· Strong knowledge of IDD, mental health, or TBI populations, 6400 regulations & ODP standards

· Proven leadership and coaching ability

· Highly organized, proactive, and solution-focused

· Comfortable working independently and making decisions

· Tech-savvy (Microsoft 365 Suite, HCSIS/EIM)

Requirements

· Bachelor’s degree in Social Work, Human Services, or a related field, plus 4 years of experience, including 2 years of management experience OR

· Associate’s degree in Social Work, Human Services, or a related field plus 6 years of experience including 2+ years of management experience

· Valid driver’s license

· Willingness to be flexible with your schedule (some evenings/weekends as needed) and be on-call as a resource for residential supervisors
