Job Responsibilities:

• Evaluate, diagnose, andtreat patients with physical impairments in a SkilledNursing Facility setting
• Develop and implementindividualized treatment plans based on patient needs andclinical findings
• Supervise and collaborate withPhysical Therapist Assistants to ensure effective caredelivery
• Monitor patient progress and modifytreatment plans as needed
• Educate patients andcaregivers on mobility, safety, and home exerciseprograms
• Maintain accurate and timely documentationin compliance with facility and regulatory standards

Requirements

• Active state license as a Physical Therapist in California
• Current BLS/CPR certification
• Minimum of 1 year of recentPT experience preferred, ideally in a SNF or geriatricsetting
• Strong clinical reasoning and leadershipskills
• Ability to work collaboratively within aninterdisciplinary team
