Registered Nurse (RN) – Community-Based Care

Location: East Moline, IL 61244

Schedule: Monday–Friday, 9:00 AM–5:00 PM, hybrid, with significant flexibility based on program and individual needs

Salary: $90,000–$100,000 annually

Position Overview

Avento Health is recruiting a Registered Nurse (RN) for a full-time, hybrid, community-based nursing opportunity in East Moline, Illinois. This position is ideal for an RN interested in combining clinical nursing with medication oversight, care coordination, staff education, and health advocacy outside of a traditional hospital bedside setting.

The RN will provide nursing care and health support services for adults with intellectual and developmental disabilities while serving as a clinical resource to direct-care staff. The position involves ongoing collaboration with healthcare providers, pharmacies, families, guardians, and other members of the care team.

Key Responsibilities

• Provide nursing care and health support services in accordance with professional standards and applicable regulatory requirements.
• Assess individuals when illness, injury, or changes in physical or mental condition occur and provide appropriate nursing guidance.
• Participate in a rotating one-week on-call schedule, providing remote/phone clinical guidance regarding medications, injuries, and other clinical concerns. On-call coverage does not require reporting to a physical location.
• Train and oversee direct-care staff in medication administration and individual health-support needs.
• Provide education regarding medical conditions, seizure disorders, specialized diets, choking risks, adaptive medical equipment, and other individualized health needs.
• Coordinate medical, laboratory, health, and dental appointments and related follow-up needs.
• Administer scheduled and PRN medications when needed in accordance with written physician orders.
• Review physician orders and medication records for accuracy and completeness.
• Coordinate medication orders and prescription needs with pharmacies and healthcare providers.
• Monitor for changes in physical health, mental status, medication response, and overall condition.
• Complete required nursing assessments, health-risk documentation, medication-related assessments, nursing notes, and other clinical records accurately and on time.
• Communicate changes in condition to appropriate healthcare providers, care-team members, families, or guardians.
• Support compliance with applicable state, federal, and quality requirements.
• Conduct CPR and First Aid training when assigned and appropriately certified.
• Provide nursing guidance during urgent situations and participate in crisis response and problem-solving as needed.

Qualifications

• Graduate of an accredited nursing program.
• Current Illinois Registered Nurse license.
• At least 1 year of RN experience.
• Valid driver’s license and current automobile insurance.
• Must possess or be able to obtain an Illinois Registered Nurse-Trainer certificate.
• Strong nursing assessment, clinical judgment, communication, interpersonal, and documentation skills.
• Ability to provide effective education and clinical guidance to direct-care staff.
• Ability to work independently while collaborating with an interdisciplinary care team.
• Ability to travel locally as required by the position.
• Ability to obtain required first aid, CPR, medication-administration, and related competencies within required timeframes.

Preferred Background

Experience in intellectual or developmental disabilities, community nursing, residential services, home health, behavioral health, case management, care coordination, long-term care, rehabilitation, chronic disease management, medication oversight, or staff education may be especially relevant.

Schedule & Work Environment

The standard schedule is Monday through Friday, 9:00 AM to 5:00 PM. This is a hybrid role combining remote work with in-person community-based responsibilities. The role offers significant flexibility in how the workday is structured, allowing the RN to adjust hours as needed to accommodate appointments, program priorities, individual needs, and other clinical responsibilities. RNs rotate one week of on-call coverage at a time; while on call, they provide remote/phone clinical guidance and do not report to a physical location.

The position may require extended periods of sitting and infrequent lifting or moving of objects weighing up to 25 pounds. The work environment may occasionally involve interaction with individuals experiencing aggressive, threatening, or substance-related behaviors.

Compensation & Benefits

The anticipated salary range is $90,000–$100,000 annually.
