Therapist - Social Worker

Job Description

Job Summary:
Seeking an experienced Social Worker to provide acute inpatient case management and discharge planning within a large hospital setting. The ideal candidate will efficiently manage a high patient caseload while collaborating with multidisciplinary teams to coordinate patient care.

Responsibilities:

• Evaluate patients' discharge needs and create comprehensive discharge plans.
• Collaborate with healthcare teams, including physicians and therapists, to optimize patient outcomes.
• Document patient progress and case management activities accurately in Epic EMR.
• Manage a caseload of 15-20 patients per day, including coordinating with community services.
• Adhere to safety protocols such as flu vaccine requirements and fingerprinting procedures.
• Quickly adapt to minimal training on the unit and work efficiently in a fast-paced environment.

Qualifications:

• Active LMSW or LCSW license required.
• Valid MS State License to practice social work.
• Minimum of 2 years of acute inpatient case management experience, including at least 1 year in a large (500+ beds) Level 1 academic medical center.
• Proven experience in acute care discharge planning and Epic EMR proficiency.
• Recent (within 2 years) inpatient case management experience in a trauma hospital preferred.
• Ability to work rotating 36/48-hour shifts and manage a high patient volume.
