Job Summary We are seeking a motivated and compassionate Collaborative Physician to join our team at Anpex Health in Hampton, VA. As a key member of our healthcare team, you will play a vital role in delivering exceptional patient care, collaborating with healthcare professionals, and promoting a culture of wellness and excellence. About the Role As a Collaborative Physician , you will provide high-quality, patient-centered care to patients in skilled nursing facilities and assisted living facilities. You will work closely with an interdisciplinary team to develop and implement individualized care plans, ensure seamless transitions, and provide ongoing support to patients and caregivers. Responsibilities Provide primary, palliative, and chronic care management to patients in skilled nursing facilities and assisted living facilities. Collaborate with healthcare professionals, including physicians, nurse practitioners, nurses, and social workers, to develop and implement individualized care plans. Conduct patient assessments, write orders, and provide medication management. Ensure timely and effective communication with patients, families, and healthcare professionals. Participate in care conferences, team meetings, and quality improvement initiatives. Stay up-to-date with industry developments, best practices, and regulatory requirements. Requirements Collaborative Physician experience in a similar setting (0+ years of experience). Active medical license in the state of Virginia.
