Case Manager

Summary Performs complex case management work. Work involves reviewing and tracking case management activities; and assisting in developing program goals, objectives, and procedures. May assign and/or supervise the work of others. Works under limited supervision with considerable latitude for the use of initiative and independent judgment. Essential Functions Maintains a working knowledge of community resources; Coordinates referrals to community agencies and tracks outcomes of referrals; Completes client assessments and care plans by identifying psychosocial, economic and physical needs of clients; Documents all client encounters; completes and submits monthly reports; maintains comprehensive electronic client files, which include client notes, release of information, assessments and other medical documents acquired on behalf of the client; Reviews encounters and records of community prevention workers; Identifies problem areas, service gaps, and areas that are barriers to services in the Lubbock community; Coordinates organizational and community groups that are working to address social determinants of health; Develop and implement educational programs; Assists with the development of policy and procedures; May supervise or coordinate the work of community workers; Plan goals and objectives for assigned programs; Assist in budget preparation; Assist with health department social media; Additional duties as assigned. Qualifications Experience in case management or social services work. Graduation from an accredited four-year college or university with major course work in social work, behavioral sciences, social sciences, public health or related field is preferred. Valid Texas Driver's License Required Knowledge and Abilities Knowledge of local community resources for social service, mental health and healthcare; Skill in developing, implementing and monitoring service and treatment plans; Knowledge of current principles and practices of planning and program implementation; Knowledge of organization and administration of public health and/or human service programs; Knowledge of program monitoring and policy analysis; Skill in communicating effectively orally and in writing; Skill in interviewing; Skill in organizing and developing programs independently; Skill in establishing effective and constructive working relationships; Skill in use of personal computers and related software applications, including e-mail; Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community; Physical Requirements Occasionally stand and walk; Frequently flex upper truck forward; Occasionally lift up to 10 pounds; Occasionally push and pull objects; Occasionally flex upper trunk forward, partial flexing on knees; Occasionally reach above, at or below shoulder height; Continuously talk, see and hear. #J-18808-Ljbffr
