SNF/Outpatient– Trinity Rehab
Location: Newton, NC
Trinity Rehab is seeking a dedicated and compassionate Speech Therapist (SLP) to join our team in an outpatient setting at a highly regarded continuing care community in Newton, NC.
Our facility is recognized for its excellence and offers a comprehensive continuum of care, including independent living, assisted living, long-term care, short-term rehabilitation, and outpatient services . - Evaluate and develop individualized treatment plans for patients
- Provide evidence-based physical therapy interventions
- Collaborate with interdisciplinary healthcare team members
- Educate patients and families on treatment goals and progress
- Maintain accurate and timely documentation
