About This Role The Patient Transfer Access Center RN will lead critical system-wide coordination efforts, utilizing clinical expertise to efficiently match patients with the appropriate site of care. Highlights Location: Somerville, MA Facility: Mass General Brigham Incorporated Schedule: Part-Time, Evening and Night shifts Pay: $70,345.60 - $136,333.60/Annual What You’ll Do Facilitate and coordinate with hospital bed management teams to facilitate patient movement and placement. Elicit patient information from referring MDs and outside facility nurses for appropriate triage and placement. Monitor the notification process for inpatient responding clinicians and appropriate documents. Serve as a department representative and consult with administrators and patient care unit leadership. Make critical decisions on directing patients to appropriate care pathways. Identify patients safe for standardized transfer pathways to reduce ED load and inpatient bed capacity.
