The role emphasizes patient advocacy, care management, and efficient resource utilization, ensuring a seamless continuum of care tailored to each patients unique needs./ph3Key Responsibilities:/h3ulliCoordinate care needs for assigned patients and develop comprehensive care plans across the continuum of care./liliPartner with patients and primary physicians to create customized, needs-based care strategies./liliCollaborate with physicians, nurses, social workers, and other healthcare professionals to ensure timely delivery of tests and treatments./liliAdvocate for patients to ensure their care needs are met efficiently and effectively./liliBalance clinical care with financial considerations to optimize treatment outcomes and resource utilization./li/ulh3Qualifications Requirements:/h3p Education:/pp- Associates of Nursing (ASN) required./pp- Bachelor of Nursing (BSN) preferred./pp Licensure Certifications:/pp- Active Registered Nurse (RN) license in Indiana or Nurse Licensure Compact (NLC) RN license./pp- Basic Life Support (BLS) certification through the American Heart Association (AHA)./pp- Additional advanced life support certifications may be required per department policies./pp- Specialty certifications may be required depending on department/provision of care./pp Experience Knowledge:/pp- 3-5 years of clinical work experience preferred./pp- Knowledge of personal computers, word processing, and database software./pp- Understanding of medical terminology, medical record requirements, regulations, and policies./pp- At least 3 years of cardiac nursing experience strongly preferred./p/div
